verify that the provider participates in our network and serves your
selected Care System. You can do this by contacting the provider's office
directly or by calling Customer Service at the phone number listed on your
member ID card.
About Patient Choice's services:
Patient Choice does not provide health services or practice medicine, nor is
it a health plan. Instead, the company arranges for providers of health
services to participate in a Care System network. Employers make the Patient
Choice network available to their employees (and their family members) as
part of their employer-sponsored health plan. Network providers are not
employees of Patient Choice. They are independent contractors.
